Neuroinflammation in Obsessive-Compulsive Disorder: Sydenham Chorea, PANDAS, and PANS

CLINICAL CARE POINT
“It is imperative to not only treat the psychiatric symptoms but to also treat active and clinically apparent infections. In some cases, postinfectious inflammation may need to be addressed with immunomodulation. In the case of Sydenham chorea (SC), pediatric autoimmune neuropsychiatric disorders associated with streptococcal infections (PANDAS), and post-strep pediatric acute-onset neuropsychiatric syndrome (PANS), eliminating Group A Strep from both the patient and close contacts…”

“This article aimed to outline current research that provides evidence of an association of PANDAS and PANS with inflammation, immune dysregulation, and/or autoimmunity.” The review article:

  1. Discusses Sydenham Chorea as an accepted model of a post-infection autoimmune response leading to neuropsychiatric symptoms.
  2. Reviews the diagnostic criteria for PANDAS and PANS
  3. Outlines the accumulating evidence that neuroinflammation plays a role in PANS/PANDAS
  4. Provides treatment recommendations and an agenda for future research
